NHS Greater Glasgow and Clyde is committed to working with patients and carers to ensure that everyone receives the best treatment possible and are engaged in decisions regarding their health and the health of those they support.
To help achieve this we should aim to identify carers as soon as possible, involve them in care and discharge planning and support them to continue to care if they choose to do so. We can refer carers to their local Carers Centres and signpost them to our Support and Information Services if they require additional support.
Remember health boards now have a legal duty to involve carers, including young carers, in discussions and decisions about the person they care for before they are discharged from hospital.
