Is the person you care for in hospital?

The Carers (Scotland) Act 2016 gives carers the right to be involved in decisions regarding the discharge planning of the person they care for. This is to help ensure that patients are discharged safely and that carers receive the support they need in order to continue to care if they choose to do so. 

We want to make sure that all our patients are supported while they are in hospital and when they leave. Friends and family play an important role in this and we want to work with everyone to make sure that patients receive the best care possible.

Below you will find information about different things to think about while someone is in hospital. This will help us work with you to create care plans, as well as discharge plans for the patient.
