Please note that when you attend you will be asked to complete COVID screening questions. Please use the hand sanitiser provided and keep a 2 metre distance at all times.

Please bring your staff ID badge with you to your appointment, or if no ID badge is available the agreed letter confirming eligibility which will be available through your line manager.
