Person-centred care is providing care that is responsive to individual personal preferences, needs and values and assuring that people’s values guide all clinical decisions and where their experience of care meets or exceeds their expectations

Care Rounding

Care Rounding is key to providing person-centred care which supports a personalised approach to care assessment, planning, delivery and evaluation. It is an ideal opportunity for meaningful conversations about what matters most to a patient at that particular time as well as ensuring their safety needs are assessed and met. The chart that needs completed after each care round can be viewed here.
