Diabetes Services for Young People

NHS Greater Glasgow and Clyde are currently reviewing the service we provide to young people living with diabetes.

We are trying to gather feedback from young people on

  • how they feel/felt about moving from the Children's Diabetes Service into our Adult Diabetes Service
  • experiences of attending the Adult Diabetes Services


This survey has been sent to young people or parents/carers of a young person between the ages of 16 and 25 who is currently attending Diabetes Services in NHS Greater Glasgow and Clyde. We would be very grateful if you (or the young person attending our diabetes service) could complete the short survey below to share your views.

The feedback you provide is anonymous and will be used to inform how we develop our Diabetes Service to support young people in future. This survey will close on 22nd May 2020. Please only complete this survey once.
