If you think that Physiotherapy might be beneficial for non-stroke related issues raised then refer to local health centre via either local patient drop-in clinic times or self-referral triage phone lines.

If yes to any of the above physio related issues and not attending another therapy service e.g. day hospital, then consider referral to the Community stroke team.

Only refer if symptoms / function have changed since last visit/ hospital discharge or last therapy session. There has to be a reasonable expectation that physiotherapy may improve the patient's situation.
