Functional Assessment


When going through this assessment, try to discuss issues with the patient regarding any new symptoms, new problems or worsening issues since hospital discharge or their previous Functional Assessment.

If this is the first for this patient, their status on hospital / Community Stroke Team discharge is included on the CDM Therapy Discharge forms sent to the practice by the AHP's.

If possible, avoid referring for expected static problems such as hemiplegic weakness unless there has been a clear deterioration on the hemiplegic, stroke-affected side, or stroke related pain has become an issue.

If your patient is already attending another AHP service e.g. Day hospital, Health Centre Physio, please do not refer to the Community Stroke Team prior to discussing with the Stroke Specialist Nurse or Community Stroke Team manager to avoid duplication.
