Please record discharge details for patients who are no longer in treatment.

No longer in treatment is defined as – not having received an OST for a 3 month period.

Please see specific guidance below regarding the use of code – 8BC0.

Use 8BC0. Treatment Complete  - to record SUCCESSFUL treatments only.

Use 949..  Died (record place of death) – if patient has died. Please ensure you record place of death.  

If the death was unexpected then consider the completion of a Significant Event Analysis (SEA) within your practice and discuss this with your named addiction worker who may consider completing the NHSGG&C Rapid Alert Briefing Note.
