• Code used is 585f. Echocardiogram shows left ventricular systolic dysfunction
• Ask about all new echo investigations in the last year and record date if not already in the system.
• If an echocardiogram was done or declined, record the date of the echocardiogram (or offer of echo) – NB. NOT today's date.
• Ask about and record ALL new investigations/interventions since last visit (if not already recorded).
• Please ensure that all patients have had appropriate investigation and specialist review, as follows:

If the patient had their first diagnosis of LVSD as an outpatient within the last year, with no in-patient stay since their diagnosis date, they require to have had in the last 15 months:
1. an echo showing LVSD (or documented that they declined echo on a specific date)
2. and a cardiology OP appointment
