Activities of Daily Living

Is the patient’s life affected by their symptoms ? (Cough, breathlessness, chest pain, oedema).

Click box to record any difficulty. Use as many as apply.

If so, ensure you have made a referral for support.

The purpose of this part of the template is to identify patients with SEVERE breathing problems who require HELP doing something. It is our job to ensure that the help is available.

All of the Activities of Daily Living available on the template suggest that help is required or that the person is dependent on someone.

For example, only tick the box where patient "has difficulty going upstairs" when your patient REQIRES HELP OR ASSISTANCE walking upstairs, but does not have that help in place.

Nurses should only choose this code if patient actually needs some kind of help (like a new bannister rail or a stair-lift), and therefore they should refer the patient to get that help.
