Why raise the issue of smoking?

  • Smoking remains the biggest single preventable cause of ill-health and premature death in Scotland, accounting for almost one in four of all deaths. It is a key factor in health inequalities with smoking rates almost four times higher than among the most affluent communities.
  • Smoking substantially increases the chances of developing:
    • Heart disease
    • Lung cancer 
    • Emphysema & bronchitis (COPD) 
    • Strokes
    • Type 2 diabetes, among many others
  • Stopping smoking: 
    • Halves the risk of a heart attack after 1 year, compared to a smoker. 
    • Halves the risk of a stroke after 2 years, compared to a smoker.
    • Can reduce the severity of COPD symptoms and is more effective than known pharmacological treatments.
    • Could leave your patient up to £250 better off* after 28 days

*based on 20 a day smoker at current September 2015 figures.

How to raise the issue and establish smoking status

  • There is no standardised tool for assessing smoking status. First establish patients smoking status, whether they smoke at all or not.
  • By asking for this information at the same time as other health topics it may help to put that conversation into the wider context of the patient's health and well-being. "I'm concerned that your smoking..."
  • It is important to try to establish rapport and empathy with the smoker. You can do this by listening, not trying to persuade, and by avoiding judgemental responses to their statements and answers.

Cigarette Equivalence

Pipe or roll-your-own smokers:
25 grams
tobacco (1oz) per week = 50 cigs/week = 7 cigs per day

50 grams tobacco (2oz) per week = 100 cigs/week = 14 cigs per day

75 grams tobacco (3oz) per week = 150 cigs/week = 21 cigs per day

100 grams tobacco (4oz) per week = 200 cigs/week = 28 cigs per day

125 grams
tobacco (5oz) per week = 250 cigs/week = 35 cigs per day etc. 

Cigar smokers:

One Café Crème (or similar small size cigar) = approximately 1.5 cigarettes

One Hamlet
(or similar medium size cigar) = approximately 2 cigarettes

One Havana
(or similar large size cigar) = approximately 4 cigarettes

Referral to Services
  • Referral by a GP practice to Smokefree services increases the chance of a smoker attending and becoming Smokefree. (Glasgow Centre for Population Health, University of Glasgow, Professor Ken Judge et al, 2006)
  • Smokefree Services staff can offer the best tried and tested way to quit, along with free Nicotine Replacement Therapy or Champix (if suitable) and  can support your patients for as long as they need it.
  • Over 150,000 people have used Smokefree Services in the last 5 years.
