Starting the Health Behaviours and Life Circumstances section of the consultation
- The Chronic Disease Management (CDM) programme, in NHS Greater Glasgow and Clyde, provides an opportunity for people with Long Term Conditions, and health professionals, to work together to not only to optimise clinical care, but also to think about their strengths and abilities, identify their information needs and the changes they can make in their lives to take control, reach their goals and maintain their health and wellbeing.
