Diabetes Structured Patient Education

All patients with Type 2 Diabetes can access group education which can help with the self-management of their condition.

Ensure any patient with a new Type 2 diagnosis has been given information to let them know about the groups and what they offer.

Offer a referral to structured patient education to any patient with a new Type 2 diagnosis THIS YEAR.

Groups are held in community venues - the coordinator will contact your patient to arrange the most convenient venue once referral received.

Newly diagnosed patients should also be given a copy of the My Diabetes Handbook which contains a wealth of useful information about diabetes for patients.

Ensure that patients are either given a paper copy (copies available from PERL) or are signposted to the electronic online version, available here.

Patients with an existing type 2 diagnosis who have not received any education in the past should also be offered a copy of the My Diabetes Handbook as per above.
