What does your data tell you?

Care bundles are evidence based, spread over a patient’s journey and have 95% reliability. The key outcome of a care bundle is the compliance % which measures the level of compliance against all measures for all patients. Thus when sampling your selected 10 patients this will highlight how you are performing in relation to DMARD care. Safe practice exists when >90% bundle compliance is achieved.

Your monthly rates of compliance will provide you with evidence which will allow you to ask critical questions such as; What outcomes you we aiming for? What DMARDS processes within our practice can we improve on? How can we develop a safety culture that engages with patients to support the delivery of safe and reliable care within our practice? Improvement model, tools and resources will be provided to facilitate and promote reflective learning and improvement.
