Active Clinical Referral Triage or ACRT is now a model being widely adopted by services to help manage patient flow. It includes use of phone and video call for information collection or pre-assessment and allows consideration of what next for the patient. In many instances a face to face assessment is not routinely required saving your patient and family unnecessary travel to a hospital site.

Can you collect clinical information using questionnaires via email or apps such as O365 Forms

Can you send out patient information leaflets or activity/exercise programmes using email ?        

(Review the GGC email policy to ensure safe use) 

Would your service information lend itself to a website hosted by NHSGGC similar to this one for your patients to access? 

Can your electronic patient record assist workflow such as templates and letter build, outcome vetting drop down box?


Near Me is now available throughout the estate with a rapid roll out of devices taking place in 2020 in response to the pandemic. Many clinic areas are now set up with at least one if not more kit - this includes a second screen, camera and speakers.

Future Developments:

vCreate - allows secure sending and storage of video capture. Initial testing in progress. 

Group work using MSTeams - several pilot sites in progress with AHP services. 

NearMe roadmap includes possibility of up to 30 on one call enabling group work.
