IDL Process – Last-minute Changes

Doctor / Prescriber steps

There are two possible scenarios:

  1. Ward Discharge has not been completed (IDL Pathway still active)
  2. Ward Discharge has been completed (IDL Printed and Pathway inactive)

1. Ward Discharge has not been completed (IDL document not yet generated)

To revise the Immediate Discharge Letter

  1. In the patient’s record in Clinical Portal click the Medication/IDL tab
  2. From menu on left click IDL Pathway
  3. Click AdHoc Tasks
  4. Click Revise IDL Form
  5. Enter a reason and click Complete. This will retrigger the IDL Form Task.
  6. Click IDL Form
  7. Make the required changes and click Complete. This will retrigger the Ward Discharge task.

To revise the Medications

  1. In the patient’s record in Clinical Portal click the Medication/IDL tab
  2. Click AdHoc Tasks
  3. Click Revise Discharge MR
  4. Type in a reason and click Complete. This will retrigger the Discharge Medication Review task.
  5. Click Discharge Medication Review then click the blue Discharge Medication Review Task button
  6. Select each medicine in the Previous Medications section and click Modify / Stop / Withhold / Continue as appropriate
  7. Make sure you carefully check every medicine to determine whether the patient should be discharged on this medicine, and that the dose and other details are correct
  8. Add medications if needed by clicking Add Medication and filling in the required fields
  9. Once a decision has been made on every medication, click Complete. This will send the medications back to Pharmacy for review.

2. Ward Discharge has been completed (IDL document has been generated)

Informing the GP of the changes

  1. In the patient’s record in Clinical Portal click the Medication/IDL tab
  2. Click Amend IDL
  3. Select the relevant IDL from the list
  4. Under Amend IDL at the bottom of the screen enter the relevant comments (e.g. change to the content of the letter or change to the discharge medications) in the Revisions Comments box then click Complete
  5. The IDL will now be re-sent to the GP with a cover sheet containing your comments

Updating the Medications

This step is to update the patient’s record for any subsequent admissions. Medication changes after the IDL has been generated are NOT automatically sent to the GP. The GP should be informed of any changes using the Amend IDL steps above.

  1. If there was a change to the medications they should be recorded in the system
  2. In the patient’s record in Clinical Portal click the Medication/IDL tab
  3. From menu on left click Medication
  4. Click Full Review
  5. From Review Type drop-down select Discharge Review
  6. Select each medicine in the Previous Medications section and click Modify / Stop / Withhold / Continue as appropriate
  7. Make sure you carefully check every medicine to determine whether the patient should be discharged on this medicine, and that the dose and other details are correct
  8. Add medications if needed by clicking Add Medication and filling in the required fields
  9. Once a decision has been made on every medication, click Complete
