Making the Most of My Medicines (MMy)

The MMy Medicines concept aims to bring a number of benefits to the management of patient’s medicines as they transfer between primary and secondary care settings  including: 

  • more fully utilising patients’ own medicines on admission, during their stay in hospital and on discharge
  • in hospital providing patients with their own supply of medicines, stored in a personal, lockable bedside cabinet
  • maximising clinical pharmacists’ contribution to patient care
  • reducing unnecessary duplication of work within hospital pharmacies
  • reducing medicine wastage on transfer between primary and secondary care
  • linking pharmacists in the community and hospital settings into a systematic approach to pharmaceutical care across the whole patient journey
  • enhancing the quality of pharmaceutical care provided post discharge by monitoring prescriptions for 2 complete repeat cycles
