Patient and Carer Forum: Long Term Conditions – 'Your Voice Counts’

There are a range of ways that you can become involved in having your say about services and care for people with long term conditions within NHS Greater Glasgow and Clyde.

The Patient and Carer Forum is there to help you.

We are a network of people affected by long term conditions (e.g. heart disease, diabetes, arthritis, stroke, chest/lung conditions) within NHS Greater Glasgow and Clyde. The Forum is linked to ‘Managed Clinical Networks’(MCNs). These bring together staff from hospital and community services, with people affected by long term conditions.

As a network of people affected by long term conditions, we aim to:
• Help the views and experiences of people affected by long term conditions be heard;
• Help make people aware of changes and improvements in health and care services for long term conditions;
• Use our experiences of health and care services and living with long term conditions to influence discussion and changes.
