Homeless Mental Health

Homeless Mental Health Team (HMHT)   The Homeless Mental Health Service is based at Hunter Street. Service Remit The service will work with people who have longer lasting conditions across all mental illness. Aim of Service To provide direct access to mental health services for people experiencing homelessness. The team works with people with both severe and enduring and mild to moderate mental health problems.   The team provides individual mental health assessment and associated diagnosis, treatment, advice, information and support and the monitoring of symptoms and side-effects of medication. They also provide targeted interventions e.g. anxiety management. Engagement Emergency referrals are seen on same day 48 hour response rate for urgent referrals 7 day response rate for non-urgent referrals The team provides direct access to homeless people and provides assessment and treatment of the whole spectrum of mental health problems. Services are delivered in emergency & temporary accommodation units and flats and bed and breakfast accommodation as well as in the premises of voluntary organisations.  Many clients are difficult to engage due to their chaotic and often transient lifestyles. The service operates on a pro-active, assertive outreach basis. Clients’ presenting problems are often complex and there is a high level of co-morbidity within the client group i.e. addictions, mental illness and challenging behaviour.   Staff Group 0.5 Consultant Psychiatrist 1 Nurse Team Leader 6 CPN's 1 Health Care Assistant 1 Counsellor Admin Support
