Homeless Families

Homeless Families Health Care Team (HFHCT)

We are a dedicated Primary Care Team working in partnership to assess, support and empower families and pregnant women staying in temporary homeless accommodation.  Together we aim to access all services and support required to improve health.

We are a team of Health Visitors, Midwife, G.P, Support Nurses, Nursery Nurse and Care Assistant.

How can we assist Families?

  • We can visit you and complete a full health assessment.
  • We work with other agencies to find GP's and schools in your local area.
  • We can offer advice on benefits.
  • We can help obtain costs towards school transportation
  • We can provide healthcare for you whilst you are in temporary accommodation.
  • We can help to maintain your links with mainstream health services.

How can the HFHCT  be contacted?

Although most of our referrals are from the local authority community case work teams, we are also happy to accept referrals from other agencies and service users.

The service is available Monday to Friday from 9.00am to 5.00pm.
