Sample Requesting Forms
All samples received in the laboratory have to be accompanied by a sample request form. This should preferably be either an electronic Trakcare form, or a blue Yorkhill Biochemistry form.
Yorkhill Biochemistry Forms
These forms should be completed giving as much detail as possible including the requesting hospital/GP clinic, the CHI number, surname, forename, date of birth, address, ward, and consultant. Details about the sample should also be given including date and time obtained, specimen type, and the tests required. Finally, the recent clinical details should be included, and the form should be signed and dated by the requesting clinician.
If you have access to the Trakcare system, then a Trakcare request should be created for specimen requests.
