Adding an SSI Form

The Site Specific Information (SSI) form provides the R&D office with specific details of the resource involved in the research project at a specific site. Every study requires an SSI form to be completed, for multi-centre studies an SSI for each site can be generated within the one project. To complete an SSI:

  1. Select R&D form on the left hand panel on the ‘Navigate’ page
  2. On the main window, select the ‘Add SSI’ tab
  3. Select either NHS-SSI (for NHS sites) or non-NHS SSI (for non-NHS sites)
  4. A table with the SSI’s will appear at the bottom of the page and the required form can be selected and populated by clicking on the name
  5. Complete the additional information required in the SSI
  6. ‘Lock’ form
  7. Submit for authorisations
