If a patient answers YES to any of the questions in section 1 of the Questionnaire they are not suitable to directly access the Vitality programme and therefore will be directed to discuss this with their healthcare professional i.e. Practice Nurse, GP or physiotherapist who should discuss their symptoms, and if appropriate, their suitability to participate in a physical activity programme.
Section 1 of the Vitality S-Q screens participants who have any contraindications to exercise and also identifies participants who have cardiovascular disease (CVD).
Staff should use their medical expertise when discussing any of this with their patient and therefore should review their symptoms and/or medical condition. The patient is not suitable for a community based physical activity programme until they are stable or have been reviewed by their GP or cardiologist as appropriate. Staff may want to consider NHS Greater Glasgow and Clyde Rehab services where appropriate e.g. Cardiac, Pulmonary or Stroke.
In addition patients with a heart condition and/or a Stroke / TIA can only access the Vitality classes via the Live Active Referral Scheme. This ensures that appropriate pre exercise screening is undertaken to enable a suitable exercise prescription and tailoring of activity for patients is undertaken. Once patients are referred to the Live Active Referral Scheme they can then access Vitality classes if suitable.
