Investigation of neonatal herpes: sample requirements

TrakCare item: 'Herpes simplex / varicella zoster virus PCR' - EDTA blood & skin swab/s & throat swab & rectal swab/faeces
AND
TrakCare item: 'Eye PCR screen - viral / chlamydia' - eye swab

Consider: TrakCare item: 'CSF viral PCR screen' - cerebrospinal fluid

Respiratory Sample Requirements

Please note that Viral PCR Sample solution (VPSS) is required for all swabs submitted for PCR testing, which includes throat (or nose/throat) swabs for respiratory viral PCR. This will provide results for:

  • Influenza A (and the H275Y oseltamivir resistance mutation)
  • Influenza B
  • Respiratory syncitial virus (RSV)
  • Human metapneumovirus
  • Parainfluenza-1/-2/-3/-4
  • Coronavirus
  • Rhinovirus / enterovirus
  • Adenovirus
  • Mycoplasma pneumoniae

Nasopharyngeal aspirate: Collection container or a universal container

If Pneumocystis jirovecii or CMV testing is required - best sample for sensitivity (to the worst sample for sensitivity)

  1. Bronchoalveolar lavage (BAL) - universal container
  2. Induced sputum - universal container
  3. Sputum - universal container

EDTA blood (pink topped vial, approximately 0.4mL of blood per test): CMV/EBV/adenovirus quantitative PCR, Human herpesvirus-6/-7 (HHV-6/HHV-7), HSV IgG, VZV IgG, CMV IgG, EBV VCA IgG, measles IgG, mumps IgG, Toxoplasma gondii IgG, syphilis IgM/IgG, HBsAg, HCV antibody, HIV antibody/antigen, HTLV-1/-2

Clotted blood: Toxoplasma gondii dye test (performed by Scottish Toxoplasma Reference Laboratory, Raigmore Hospital, Inverness)

Urine: Universal container

Faeces: Universal container - This will provide results for:

  • Norovirus
  • Sapovirus
  • Adenovirus
  • Astrovirus
  • Rotavirus

If faeces is unavailable a rectal swab in VPSS can be submitted for testing.

CSF: Universal container - This will provide results for:

  • Herpes simplex virus-1 (HSV-1)
  • Herpes simplex virus-2 (HSV-2)
  • Varicella zoster virus (VZV)
  • Enterovirus
  • Parechovirus

Also available on request:

  • Human herpesvirus-6/-7 (HHV-6/HHV-7) 
  • CMV/EBV/adenovirus
  • JC virus
