Heart Disease

AIM OF MCN

To improve the Heart Health of the people of Glasgow; and to ensure that the adult population of Glasgow has equitable access to high quality cardiac services, which are based on evidence, best value for money principles and patient preference.

OBJECTIVES OF MCN
  • To develop and implement a Glasgow Heart Disease strategy, which takes into consideration all aspects of the patient pathway from prevention to chronic disease management.

  • To ensure that the Network promotes multi-disciplinary and inter-agency working - pivotal to its success will be the involvement of patients, carers, local authorities and the voluntary sector in developing and planning preventive and care services.

  • To review and, as required, re-design adult cardiac services to ensure equity of access to high quality, evidence based services, while acknowledging potential financial limitations.

  • To develop a health improvement agenda for Glasgow, which will seek to use innovative and creative ways to assist the population in contributing positively to their heart health.

  • To review existing, and develop new, guidelines and protocols for the management of heart disease consistent with national guidelines.

  • To ensure integration of these into the local health planning process.

  • To establish a system of monitoring and reviewing the clinical effectiveness of cardiac services in Glasgow – to deliver, at least, the NHS QIS standards.

  • To develop IT services (including an EPR) within the overall national and local IT strategy, which will assist direct patient care, minimise clerical chores and deliver relevant, timely audit data as required by national bodies and for local service improvement.

  • To ensure that local and national waiting time targets are met for out-patient appointments, investigation and intervention. To acknowledge and deal with other delays in the patient journey.

  • To ensure that information about heart health and health care is available in accessible formats for those who wish to access it.
