Setting up the project :-

 

Why the project was set up?

As occupational therapy service worked across 2 CHCP’s there was a need to provide core skill intervention which was evidence based and meeting needs of individuals referred to the service and maximising use of occuptational therapy resources.

The group was set up to offer an evidence based group therapy for patients experiencing early stage cognitive impairment. The therapeutic approach provides the opportunity for ongoing assessments and individual goal setting,,activity planning and peer review.

What are the aims and objectives?

Aim: To limit the occupational impact of cognitive impairments by promoting independence and engagement in purposeful activity.

 

Objectives: To provide a safe and supportive environment where group members can explore the impact of cognitive impairment on their current level of occuopational performance and identify areas where compensatory techniques can be applied to minimise impact of  occupational performance deficits.

 

To identify individual goals to maintain and prolong levels of independence when engaged in familiar and routine activities of daily living.

 

Who was involved in setting it up?

( Debbie Scott)Lead occupational therapist, 2 senior occupational therapists (Kevin Jamieson, Dorothy Thomson) devised and designed  first group process and 2 other occupational therapists ( Jennifer Conway & Lindsey O’Hare) and 1 technical instructor ( Anne McDonald) are currently undertaking review of evidence base and group sessions.

 

What were the main challenges?

Ensuring staff allocated sufficient time to work on tasks required to develop the group and to view group development as a valuable use of clinical  time and resources.

Description of project :- (No more than 500 words)

Individuals are referred to the occupation and memory by members of the Community Mental health Team and Day Hopsital staff.  Patients are then visited by an occupational therapist.  The Model fo Human Occupation Screening Tool (MOHOST) is administered (carried out after week 2 and  week 6). This screening tool measures a patients occupational participation it provides a strucutre or framework by which the therapist would understand why a patient is not engaging in a range of  every day occupations.

The group process consists of :

-          7 weekly sessions of approximately 2hours in duration.

-          Sessions are facilitated by 2 occupational therapy staff

-          Individual activity log with weekly goals

-          Therapeutic (closed) group format to utilise peer reinforcement and communication/interaction skills

-          Use of the MOHOST at 2nd and 6th week during group attendance as outcome/evaluation tool.

-          Follow up home visit after cessation of group process to monitor transference of skills outwith group environment.

-          Individual treament review where further individual sessions to reinforce techniques may be indictated or discharge from service.

Evaluation :-

The MOHOST completed in 2nd and 6th week.

Occupation & Memory group qualitative evaluation form carried out at end of last group session

Has an evaluation  taken place?

Occupation & Memory group evaluation form is completed after every cohort.

How do you know it has made a difference?

The MOHOST evaluations indicate positive changes in the areas of:

-          Motivation for occupation

-          Appraisal of ability

-          Occupational choices

-          Pattern of occupational performance ,routine and communication and interaction skills.

What changes would you make if you were starting the project again?

Currently undertaking a review of literature and consequently structure and content of sessions.

 

Future plans for project :-

Yearly review of literature and apply new techniques as detemined by evidence available at time of review.
