Chronic Disease Management

In recent years more and more people are successfully living with and managing chronic diseases.  Chronic diseases are diseases of long duration and generally slow progression. They include heart disease, stroke, chronic respiratory diseases and diabetes.  The care of chronic disease is delivered in both hospital and community settings and is often managed by patients themselves who become experts in their own condition and its treatment.

Scope of Review:

Current pathways of care for patients with chronic diseases, including:

  • heart disease
  • diabetes
  • respiratoy disease
  • rheumatology

The group will focus on:

  • balance of care between primary and community care and hospital care,
  • anticipatory care,
  • palliative care,
  • supporting patients at home.
